The Legal module allows you to storage all your health-related legal documents and track them over the time.
Features Included
The legal module includes the tiles below:
- HIPAA Authorization: legal document to allow you to provide others access to your protected medical records, most often to other doctors or care providers. However, this form can also be used to release your medical information to a specific person.
- Organ donation: legal document to record a decision to donate some or all of your organs, tissue or any parts to be used for transplantation, research, therapy and/or education upon your death.
- Refuse donation: legal document to record a decision to opt out of organ, tissue or any parts to be used for any purpose upon your death.
- Do not resuscitate (DNR)* **: legal document to inform medical staff in a hospital or nursing facility that you do not want cardiopulmonary resuscitation (CPR) or other life-support measures to be attempted if your heartbeat and breathing stop.
- Out-of-hospital do not resuscitate (OOH-DNR)* **: legal document to alert emergency medical technicians (EMT) of your wishes regarding measures to restore your heartbeat or breathing if you are not in a hospital.
- Do not intubate (DNI)* **: legal document to inform medical staff in a hospital or nursing facility that you do not want to be on a ventilator.
- Do not hospitalize (DNH)* **: legal document to indicate to long-term care providers, such as nursing home staff, that you prefer not to be sent to a hospital for treatment at the end of life.
- Living will* **: legal document to specify what actions should be taken for your health if you are no longer able to make decisions for yourself because of illness or incapacity.
- Living will revocation* **: legal document to cancel any previous living will.
- Physician orders for life-sustaining treatment (POLST)* **: legal document to provide guidance about your medical care that health care professionals can act on immediately in an emergency. Issues covered in the POLST advance directive include: resuscitation, mechanical ventilation, tube feeding, use of antibiotics, requests not to transfer to an emergency room, requests not to be admitted to the hospital and pain management.
- Visitation authorization* **: legal document to make sure someone can visit you in the hospital and help you bypass red tape so your friends can support you in person in the event you should be injured or fall ill or be incapacitated through any other cause that necessitates my hospitalization or treatment in a medical facility.
- Visitation ban* **: legal document to make sure someone banned by you, won't be admitted visiting you in the event you should be injured or fall ill or be incapacitated through any other cause that necessitates my hospitalization or treatment in a medical facility.
- Power of attorney (POA)* **: legal document that names your health care proxy and that ends when you become incapacitated.
- Mental power of attorney (MPOA)* **: legal document that grants a selected individual or entity permission to make mental healthcare decisions for you, such as admitting you into a facility and refusing or accepting certain treatments.
- Durable power of attorney (DPOA)* **: legal document that names your health care proxy and that remains in effect even if you become incapacitated temporarily or permanently.
- Power of attorney (POA) revocation* **: legal document to cancel any previous POA.
