Instructions

In order to generate your personal ICE Card please read carefully the following instructions below:

• Enter as much information as possible from following fields: First Name*; Last Name*; Birthdate*; Blood Type*; Major food allergies (current); Major medication allergies (current); Major environmental allergies (current); vaccines (doses); Major medication (current); Major conditions (past); Major prosthesis (current). Please note fields marked with an asterisk are mandatory.
• Enter all dates for each field entered.
• The ICE Card only allows up to 11 different inputs in the rear of the card. In case of more information entered you will be able to select and/or prioritize later all these 11 items more important for In Case of Emergency (ICE) occasions.
• Once all information will be entered and submitted, a unique token and PIN will be generated in exclusive for you and an ICE Card like the one below will be created:
